What you will do
Choose how often each problem has bothered you during the previous 2 weeks.
What the GAD-7 measures
The GAD-7 asks how often seven common anxiety symptoms have bothered you. Each item is scored 0–3 and the total ranges from 0–21. Established ranges describe symptom severity; they do not by themselves establish a diagnosis.
None–minimal symptom range.
Mild symptom range.
Moderate symptom range.
Severe symptom range.
A commonly used threshold for a positive GAD screen; diagnosis still requires further assessment.
Scores describe reported symptom burden. They do not explain the cause of anxiety or decide what treatment you need.
Scoring and interpretation
Responses are scored 0 for “Not at all,” 1 for “Several days,” 2 for “More than half the days,” and 3 for “Nearly every day.” The seven scores are added for a total from 0 to 21.
The complete score and severity range are free. We do not make someone pay to learn their basic clinical screening result.
A score of 10 or more is a commonly used screening threshold, but a screen is not a diagnosis.
What a clinician adds
A professional may consider how long symptoms have lasted, how much they interfere with daily life, physical health, medication or substance effects, sleep, depression, trauma, panic symptoms, and other possible causes.
If anxiety feels overwhelming, is worsening quickly, or makes it hard to function, professional support can be useful regardless of the exact score.
What this screen can and cannot tell you
The GAD-7 was developed to screen for generalized anxiety disorder and is also widely used as a measure of anxiety symptom severity. Other anxiety disorders, stress, medical conditions and life circumstances can produce similar symptoms.
This screening implementation should not be treated as a validated clinical or diagnostic tool unless an independent qualified reviewer verifies the exact instrument version, wording, scoring, source terms, privacy flow, and U.S.-first care guidance.
Sources and instrument governance
Pfizer has made the GAD-7 available through PHQ Screeners without copyright restriction/charge subject to the applicable source terms. Final production use still remains subject to rights and clinical-review verification.
Frequently asked questions
No. It is a screening and symptom-severity tool. Diagnosis requires a fuller clinical assessment.
Yes. The total score, severity range, limitations, and basic next steps are free with no signup or email required.
Ten or more is a commonly used screening threshold that can justify further assessment. It still does not prove that someone has generalized anxiety disorder.
Interpreting a GAD-7 result
A higher score suggests greater current generalized-anxiety symptom burden, but the same total can occur in different situations. Panic, social anxiety, OCD, trauma-related symptoms, health anxiety, depression, sleep problems, medical conditions and substance or medication effects can overlap with some GAD-7 items.
| Use the score for | Do not use the score for |
|---|---|
| Summarizing recent symptom burden | Diagnosing generalized anxiety disorder by itself |
| Deciding whether a professional conversation may be useful | Explaining the cause of anxiety symptoms |
| Tracking change when used consistently | Ruling out other anxiety or mental-health conditions |
If worry is persistent or affecting sleep, work, study or relationships, combine the result with the broader guidance in the anxiety symptoms hub. For overlapping everyday pressure, see anxiety vs stress.
